Service Description:

Community-based health care in which paramedics provide alternative to family doctor, walk-in clinic, or emergency department
- provides preventative, at-home health care visits
- focuses on reducing avoidable use of emergency and ambulance services
- helps older adults and vulnerable, at-risk people live safely in their homes
Services include:
- health and medical risk assessments
- chronic disease management
- medication management
- care coordination with primary care providers and other agencies
- referral to community programs and resources
- remote patient monitoring
