- assists Halton residents without a family doctor to connect with a primary care provider through a referral and matching process
- provides personalized support for people experiencing barriers to finding a family doctor, such as language, transportation, internet access, or system navigation challenges

- register for Health Care Connect and a nurse will search for a doctor or nurse practitioner who is accepting new patients in the local community
