Service Description: Supports for older adults, and adults with disabilities by providing personal care and homemaking services in 8 buildings in Mississauga and 2 buildings in Brampton * helps people remain independent in their own homes for as long as possible * care coordination and referral to other community services

Supports for Daily Living (SDL) - Program assists older adults with the essential activities of daily living, which include personal care, medication assistance  (receive medication reminders either via phone or in-person), safety assurance program, laundry, purchasing grocery items, light meal preparation, and light homemaking * servicing clients in 10 different buildings and also in the neighborhoods around those buildings

Supportive Housing Program - For people who require daily personal support and essential homemaking to live independently * available in designated residential buildings and also in the neighborhoods around those buildings (depending on individual need) * personal support is provided on-site and is available up to 24 hours a day, depending on individual need

Other services include:
  • care coordination
  • foot care services available at some locations (diabetic foot care and health education)
  • medication management (partnered with Resident Care Pharmacy)
