Service Description: An outpatient service adults over the age of 65 with complex medical and psychosocial issues such as depression, anxiety and other mental health illnesses. The interprofessional services include: - Comprehensive assessment and diagnosis, - Treatment options, including medication management and non- pharmacological approaches (e.g. psychotherapy, relaxation and breathing techniques) - Problem solving therapy and goal setting - Group-based counselling and therapy - Group interventions, activities and exercises

Referrals from a physician or nurse practitioner for this service can be initiated through our Central Navigation, which adheres to a no-wrong-door approach. This assists community agencies, primary care providers, and other healthcare professionals in navigating Ambulatory Services at Baycrest.
