Service Description: This program provides structured and supervised social, recreational, physical and educational activities for frail seniors, post-stroke clients and those living with dementia *  helps prevent social isolation and encourages independent living in the community as well as provides support and information services for caregivers * transportation is not included in program fee
  • caregiver support program
  • individual/group supportive counselling
  • assistance with activities of daily living
  • foot care service
  • snacks and hot lunch
  • bathing service for adult day program clients and who require one-person transfer only

Fees: $32 per day for in-person program and $8 per session for virtual program * additional fee for transportation, outings and special events
Extended hours: Additional $10 per person per day

Eligibility / Target Population: Older adults, adults with disabilities, and people with Alzheimer's Disease or related dementias who need physical, social, or emotional support * Ontario Health atHome Care Coordinators assess the needs of applicants
