Service Description: One-stop point of access of Ontario Health atHome in Mississauga Halton for:

With one phone call, a team member will help determine eligibility for these programs and connect with the care needed or refer to alternative resources that may better meet the needs

Serving the communities of Oakville, Milton, Halton Hills, Mississauga and South Etobicoke to:
  • assess client eligibility for these programs based on standardized criteria
  • prioritize incoming referrals
  • assign clients the most appropriate service provider to meet their needs
  • track and trend the demand for services
  • maintain a centralized waitlist for services

Application: Application required * online referral form available
Eligibility / Target Population: High risk seniors and adults 18 years of age and over with non-medical needs who can be effectively supported by qualified staff * has valid OHIP health card
