Service Description:
|
Provides foot care education and management to individuals with diabetes who have financial barriers to accessing fee-for-service foot care * team includes a chiropodist and a foot care nurse (nurse with specialized foot care training)
Services include:
- routine and complex nail care
- corn and callus reduction
- diabetic foot care
- early wound management
- footwear assessment and education
- foot care and self-management education

Qualified patients 18 years of age and older with diabetes residing in Mississauga Halton Region who are enrolled in a Diabetes Education Program in Mississauga/Halton
