Service Description: The role of any Supportive Housing program is to provide an alternative to Long Term Care facilities while maintaining people’s independence and integration into their communities

Support for Daily Living Program (SDL): 
  • provides residents (high needs seniors) with 24-hour assistance with activities of daily living
  • aimed at reducing unnecessary stays in hospital for seniors once their acute care phase is concluded and provides an alternative to Long Term Care facilities while enabling a senior to maintain their independence and integration in their communities
  • SDL model includes multiple daily visits based on individual need (approximately 1.5 hours per day) by trained personal support workers (PSWs)
  • services are not available to seniors residing in retirement homes, group homes or long-term care facilities 
  • volunteer opportunities available

Assistive Living Program provides attendant services to seniors 65+ who meet the eligibility requirements: Seniors' Residence at 155 Forum Dr

Personal support services include: 
  • washing and hair care
  • bathing
  • dressing/undressing
  • application of non-medicated creams or eye drops
  • transferring/positioning
  • mouth care
  • assistance with eating, toileting
  • safety checks/reassurance services
  • medication reminders and assistance

Homemaking services may include: 
  • light meal preparation/clean-up after use
  • tidying up the bathroom/kitchen/bedroom after use
  • garbage disposal
  • changing bedding
  • light housekeeping
  • laundry

Fees: Support services are free * rent geared to income
Application: Call * for subsidized housing apply through Peel Access to Housing (PATH) * for Assisted Living Program contact Intake and Referral worker ext 6 for an assessment
Eligibility / Target Population: Seniors 65 and older who require the availability of services throughout a 24-hour period and who meet the eligibility requirements
