Service Description:

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors and nurses, nurse practitioners, social workers, registered dietitians, a pharmacist, a physiotherapist, and a podiatrist (foot specialist)
Preventative Care - breast cancer screening * cervical cancer screening * colorectal cancer screening
Diabetes Education Program - Information on monitoring, blood sugar levels, food choices, exercise, medications, and lifestyle choices
Breath for Life - Provides lung function testing (spirometry), education on lung health and smoking cessation along with medication management and ongoing follow up for patients with mild to more severe forms of asthma or COPD * contact Catherine Harrison at 905-813-1100 ext 5468
Smoking Cessation (STOP) Program - One-on-one support to help quit smoking * facilitate access to nicotine replacement therapy (NRT) * 6 months of nicotine patches, gums, inhalers or lozenges may be supplied
Procedure Clinic - offers once a month for assessment and performance of minor procedures * procedures are performed by specialized staff physicians and residents of the Credit Valley Family Health Team
Nutritional Services - Infant Nutrition Program * Weight Management Program * services provided by Registered Dietitians
Mental Health - a team of family physicians, clinical social workers and registered dietitians work to address your mental health needs * 24/7 Crisis Support Peel Dufferin, call 905-278-9036 or 1-888-811-2222
Immunization - Do not provide travel vaccines

Family Health Teams are primary care organizations that include a team of interdisciplinary healthcare professionals who work together to provide comprehensive primary care to their patients
If you are looking for a francophone physician, our bilingual office is open and accepting new francophone patients * primary healthcare model offered by an inter-professional team together with a family doctor
Patients also have the option to have minor surgery done at the Family Health Team office instead of having it done at the hospital * patients are responsible to pay all costs associated with the procedure that are not covered by OHIP
