Service Description: Emotional, social and spiritual support, companionship, respite care for people and their family/caregivers dealing with a life-limiting illness * services provided at home

Hospice Care
  • hospice coordinators meet with clients and caregivers in their home to understand their physical and emotional needs * they listen, support, educate and connect to resources in the community
  • trained volunteers can provide much needed respite for caregivers, practical assistance and a listening ear
  • in-home wellness services include complementary therapies and music therapy

Provides trained staff and volunteers to support individuals who are experiencing grief and loss through the death of a loved one * volunteer opportunities and training available

Bereavement support:
  • bereavement support request available online
  • one-on-one visits
  • support groups: spousal/partner loss; adult child loss; mixed loss; virtual drop-in
  • bereavement resource library in the Oakville office
  • information on the grieving process
  • bereavement walking group in Oakville

Hospice Palliative Helpline: 905-667-1865
  • confidential counselling and emotional support by phone or video conference
  • provide/share resources
  • facilitate conversations about the goals and plan of care for end of life
  • support, information and education about grief, and spiritual distress
