Service Description: Provides community-based support for individuals who are homeless or at imminent risk of becoming homeless and are diagnosed with a serious and persistent mental illness * Trillium Health Partners is in partnership with community based organizations which provides housing and support

Services include:
  • case management
  • home visits
  • supportive counselling
  • rehabilitation assistance

Following assessment and acceptance by the program, the client is assigned a HASP worker from one of the agencies:

Applications for housing are processed by Supportive Housing in Peel (SHIP) * a screening process is conducted through their intake department * files are assigned to the appropriate partnering agency based on individual needs of the applicant * Trillium Health Partners (THP) works in partnership with community based organizations to provide housing and support

Eligibility / Target Population: People 16 years and older * with serious mental illness * has residential, family or service links in Peel * is homeless or likely to become homeless
