Service Description: The program consist of the following components:

Outbound Program: Telecheck Program for Seniors(55+)
Serving seniors, 55+, in the Ontario Health atHome-Brampton (Central West) (Brampton, Malton, Caledon, North Etobicoke), offering emotional support, regular safety check-ins, and medication reminders. By providing companionship, help is provided to clients cope with feelings of isolation and contribute to their sense of community. 8 languages available (English, Mandarin, Cantonese, Urdu, Punjabi, Hindi, Portuguese, and Spanish

Outbound Program: Caller Reassurance Program (CRP), 55+
Designed to service clients 55+ residing in Toronto (including Scarborough, Etobicoke, and North York) by providing medication reminders, safety check-ins, and social calls. This program is only available in English

Outbound Program: Touching Base (16+)
Short term support program for people over 16. Volunteers provide registered participants with medication reminders, safety check-ins, or emotional support through friendly social calls
