Service Description: Six-week program * exercise and falls prevention program for older adults with multiple medical conditions * aims to restore their functional abilities, improve their quality of life, and allow them to live independently or with the support of a caregiver in a community setting

Rehabilitative services, aim to reduce hospital visits and delay/prevent placement in a long-term care home * people who require two out of three disciplines including:
  • physiotherapy
  • occupational therapy
  • speech language therapy
Client can enroll in the program if they experience one or more of the following:
  • recent episode of acute care (hospital admission)
  • suffering from stroke, Parkinson's or other neurological disorders
  • unsteady on feet, loss of balance, strength, range of motion
  • difficulties walking
  • difficulty finding words
  • coughing while eating or taking longer to eat meals
  • gradual or sudden changes to communication, e.g., voice, comprehension
  • has difficulty or is unable to perform the activities of daily living

  • The Village of Erin Meadows, 2930 Erin Centre Blvd, Mississauga, L5M 7M4 (Tue and Fri)
  • Sheridan Villa Long Term Care Home, 2460 Truscott Dr, Mississauga, L5J 3Z8 (Mon & Wed)

Application: Physician referral not required * referral available online or call
Eligibility / Target Population: Older adults with multiple medical conditions and limited mobility (frail) * people with conditions that will improve with rehabilitative care * those who could benefit from a multi-disciplinary approach to rehabilitation
