Supports patients who have some level of kidney disease but do not presently require dialysis * focused on providing education kidney disease, lifestyle tips to help slow the progression of kidney disease, dietary support, assistance with medication management, and coping strategies * works with patient on an individual or group basis * goal is to encourage patients to be active in their own healthcare and to optimize wellness
