Service Description: Falls can cause injury, pain, loss of movement and function, loss of independence, hospitalization and even death * comprehensive assessment by a nurse practitioner and/or geriatrician, and a physiotherapist, with recommendations for treatment

Strong and Steady Program - Education and exercise sessions * participants will improve strength and balance, learn how to make their home safer, find out their risk of having a fall, and learn many ways to protect themselves * two-hour education and exercise session, twice a week for six weeks * follow-up visit three months after completion of the program

Assessments take place at the following fall prevention clinics:
  • Queensway Health Centre site, 150 Sherway Dr, 2nd Floor Medical Clinics, Etobicoke, M9C 1A5
  • Credit Valley Hospital site, 2200 Eglinton Ave W, Seniors' Services 1F, Mississauga, L5M 2N1

Exercise and education classes take place at either one of these two locations:
  • Meadowvale Community Centre, 6655 Glen Erin Dr, Mississauga, L5N 3L4 (Wednesday and Friday mornings)
  • Mississauga Valley Community Centre, 1275 Mississauga Valley Blvd, Mississauga, L5A 3R8 (Wednesday and Friday afternoons)

Application: Medical referral required
Eligibility / Target Population: Medically stable adults 65 years and older, who live in the community and want to stay strong, steady and independent * must be well enough to walk 25 meters (with a walking aid if needed) and learn new information
