Service Description:

Self-management, prevention and education rehabilitation program for patients and family members who have been diagnosed with a coronary artery disease * cardiovascular rehabilitation team includes cardiac rehabilitation therapists, clinical pharmacist, medical doctor, psychiatrist and registered dietitian
Services offered include:
- medical evaluation
- nutritional counselling
- cholesterol management
- pharmacy counselling
- heart health education
- stress management
- spouse and family support
- support to stop smoking
- personalized exercise assessment
- supervised exercise classes at Recreation, Athletic and Wellness Centre, University of Toronto, 1825 Outer Circle, Mississauga, L5L 1C6
Patients with any one or a combination of the following can be referred to the program:- coronary artery disease
- angioplasty
- heart attack
- heart failure
- bypass surgery
- heart valve surgery
- device implant
The Exercise for Your Heart video can be purchased on DVD from the Queensway Health Centre for $20
