Service Description: Self-management, prevention and education rehabilitation program for patients and family members who have been diagnosed with a coronary artery disease * cardiovascular rehabilitation team includes cardiac rehabilitation therapists, clinical pharmacist, medical doctor, psychiatrist and registered dietitian

Services offered include:
  • medical evaluation
  • nutritional counselling
  • cholesterol management
  • pharmacy counselling
  • heart health education 
  • stress management
  • spouse and family support
  • support to stop smoking
  • personalized exercise assessment
  • supervised exercise classes at Recreation, Athletic and Wellness Centre, University of Toronto, 1825 Outer Circle, Mississauga, L5L 1C6

Patients with any one or a combination of the following can be referred to the program:
  • coronary artery disease
  • angioplasty
  • heart attack
  • heart failure
  • bypass surgery
  • heart valve surgery
  • device implant
The Exercise for Your Heart video can be purchased on DVD from the Queensway Health Centre for $20

Fees: Most services covered by OHIP
Application: Medical referral required
Eligibility / Target Population: Adults 18 years and older who have been diagnosed with a coronary artery disease and their family members
