Service Description: Hospital-based program with a focus on rehabilitation * rehabilitation is offered to adults with neurological, medical, orthopedic, and cognitive disabilities * geriatric assessment is offered to the elderly person requiring assessment and inter-professional team involvement are required to enable the person to maximize functioning * both programs offer a coordinated program of assessment, treatment, and education by a inter-professional team of health professionals * program may last for several days or several weeks depending on the needs of the patient

Services may include: 
  • physiotherapy 
  • occupational therapy
  • recreation therapy 
  • social work
  • dietitic consultation
  • speech and language therapy

Unit 1D is a 55-bed rehabilitation unit (40-bed rehabilitation unit and 15-bed geriatric Sub Acute Geriatrics (SAGe) at Credit Valley Hospital)

Fees: Most services covered by OHIP
Application: Medical referral required
