Service Description: A collaborative care model integrating geriatric medicine and geriatric psychiatry anchored in primary care where the primary care provider remains the most responsible provider * medical psychiatry team includes registered nurses, an occupational therapist, a geriatrician, a geriatric psychiatrist and a primary care provider * under the supervision of geriatric specialists, RN Care Managers and allied health professionals will work with the patient and the primary care provider to manage both the mental and physical health needs with a collaborative care intervention for up to 16 weeks

Eligibility / Target Population: Seniors over 65 who live in the Mississauga Halton region with any chronic medical condition and depressed mood and/or anxiety
