Occupational therapy * support aging-in-place with home safety/accessibility assessments * offer information, referrals and care coordination regarding public and private options for home health equipment, personal care, rehab, and community services. * provide education to clients, family caregivers, and seniors' service providers on topics such as home modifications to support healthy aging, falls prevention, safe lifts and transfers, use of assistive devices, and adapting daily activities for people with physical or cognitive health changes
