Medical care team focused on family and community health * primary care services such as disease prevention management, disease treatments, rehabilitation, palliative care and health promotion * team includes family doctors, nurses, nurse practitioners, social workers, dietitian, consulting psychiatrist and psychologists, health promotion expert, and pharmacist
