Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioners, dietitians, diabetes educator, occupational therapist, chiropodist (foot care specialist), health promoter/educator, and social workers
- healthy aging for adults 55+ workshop
- health and nutrition program
- free community prenatal collaborative series
- preventative care and screening program
- pre-diabetes and diabetes education program
- mental health program
- lung health program
After Hours Clinic:
- doctor on-call * available for registered patients only
- on-call doctor holds clinic in his/her office
