Service Description: A multi-service organization, offering Children, Youth & Family Services, Housing & Homelessness Services and Newcomer & Community Services, to meet diverse community needs in Hamilton, Halton Region and Brantford

Provides essential support for individuals and families at risk of homelessness in the Halton Region, helping them find both immediate and long-term, safe, and stable housing solutions

Emergency Supportive Housing Program:
  • housing focused program supports families and women who are experiencing homelessness
  • through dedicated case management, clients are guided to secure permanent long-term housing
  • self-referrals, call 3-1-1 (Access Halton) and inquire about the Wesley Emergency Supportive Housing Program

Housing First:
  • assists chronically and episodically homeless residents of Halton in securing permanent housing while offering services and supports to ensure long-term housing stability
  • referrals to this program are made through Halton’s Homelessness Response System

Community Housing Support Worker Program (CHSW):
  • collaborates with community housing providers in Halton to address issues related to tenancy preservation
  • provides support to adults, families, and youth to prevent evictions, access necessary services, and build capacity within housing organizations
  • referrals to this program are made through Halton Region
