Service Description: Help reduce social isolation and loneliness for older adults in the community * offers programming and social opportunities for individuals who are alone and wish to be more physically and mentally active

Programs include social activities, physical activities such as sports or recreation, arts and crafts, cards and games, educational activities, health, and wellness

Fees: Free membership for one year * Subsidies are available to participate
Application: Complete referral form to be completed by healthcare professional and send via email to Bethany Vallentin at bvallentin@nyseniors.org
Eligibility / Target Population: Older adults 55 plus who are socially isolated
Languages: English
Language Notes: Language assistance depends on availability of staff or volunteers
