Service Description:

Nurse Practitioner-led multidisciplinary clinic providing safe, accessible and gender affirming care within a multidisciplinary, shared care model * team includes a Registered Nurse (RN), Registered Practical Nurse (RPN), Social Worker, System Navigator, and Pharmacist
Services include:
- Support for social, medical and legal transition processes
- Education and information on gender identity and healthcare options
- Surgical assessment, referrals and support
- Sexual health services and screening
- Hormone therapy
- Psychosocial support throughout the transition process
- Group therapy sessions and workshops
