Service Description:

A mobile and multi-agency outreach team that provides immediate support, advocacy, and referrals to people who are experiencing homelessness or at risk of becoming homeless * the team includes mental health support staff, substance use harm reduction staff, family transitional outreach staff, and housing support worker
Outreach team can help with:- Basic life essentials, including blankets, food and clothes
- Accessing Street Nursing Care
- Referrals to emergency shelter, employment supports, housing supports, mental health and addiction supports

Individuals, 16 years or over, and families who reside in Peel Region and are experiencing homelessness, or at imminent risk of becoming homeless
