Service Description: Provides 6 to 8 weeks of support to older adults (50+) with developmental disability who are either residing in or preparing to move into long term care facilities
  • staff will plan and develop a transition plan to assist a person to move seamlessly from life in the community to life in a long term care home
  • the goal is to help people maintain their connections with community involvement and participation

Fees: Depending on hours, funding and availability
Application: Application through Developmental Services Ontario
Eligibility / Target Population: Residents of the Region of Peel with a developmental disability. Individuals who currently reside or are preparing to move into a long term care facility. Adults 50 years and over
