Service Description:
|
PRC's provide education, training, and consultative support to staff at long-term care homes or adult day programs supporting older adults living with responsive behaviours/ personal expressions associated with dementia, complex mental health, substance use, and/or other neurological conditions
Delivers BSO Core Curriculum and customized education sessions based on identified needs. Assist and advise staff in the development of care plans and interventions, the use of assessment tools, and system navigation for other supports and services

Referrals can be made by anyone within a government funded agency; who has identified a learning need related to the mental health of the elderly

Eligibility / Target Population:

Must be a government funded agency such as: Ontario Health atHome; Long Term Care Homes; Adult Day Programs; Organization dealing with dementia and mental health issues
