Service Description:
|
Supports health care professionals to deliver a client-centred approach to dementia care and responsive behaviour
Supports staff that provide care for seniors with responsive behaviour related to complex physical, cognitive and mental health needs
Provides staff education for Long-Term Care and Adult Day Services to enhance:
- knowledge and skills of staff
- consultation about learning needs
- care plans and programs related to challenging behaviours of clients / residents
- assistance with referrals to services and resources and enhanced development of linkages between services providing dementia care

Referrals can be made by anyone within a government funded agency; who has identified a learning need related to the mental health of the elderly
|
|
Eligibility / Target Population:
|
Must be a government funded agency such as: Ontario Health atHome; Long Term Care Homes; Adult Day Programs; Organization dealing with dementia and mental health issues
