Service Description: Six-bed palliative care hospice providing 24-hour quality care and end-of-life care in a home-like setting * trained volunteers provide support to residents and their families, as well as perform administrative and household tasks in the House * volunteer opportunities available

Also provides one or two-week respite stay for caregivers who are providing care to a loved one who is terminally ill in their homes

Fees: Free
Application: Call * referrals may be made by individuals, physicians, medical institutions, service providers or volunteer organizations
Eligibility / Target Population: Individuals aged 18 or older with a life expectancy of three months or less who need around-the-clock end-of-life care or bereavement supports * Priority of admission is determined by a Home Coordinator on an assessment of the most urgent need
