Service Description: Apartments with 24-hour personal care * provides housing and support services to 10 adults with physical disabilities and brain injuries * 10 private bedrooms with shared common areas (dining room, kitchen, living room and recreation area)

Personal Support Workers provides assistance with:
  • Activities of Daily Living (ADLs): personal hygiene, dressing/undressing, oral care, hair care, preventative skin care, assistance with eating, exercise/range of motion, transferring/positioning/turning
  • Instrumental Activities of Daily Living (IADLs): meal preparation, housekeeping, medication assistance (limited), phone use, escort to medical appointments and transportation/shopping (where finding and resources are available)

  • have a permanent physical disability or acquired brain injury
  • be 16 years of age or older
  • have a valid health card
  • be able to direct his/her care
  • have his/her medical needs met in the community

Accredited by the Commission on Accreditation of Rehab Facilities (CARF) in 2016
