Service Description: one-Link is not a crisis service * in an emergency call 9-1-1 or go to the nearest hospital emergency department

A single point of access for referrals to publically funded addictions and mental health providers in Mississauga and Halton

Accepting referrals for clients residing in the the Mississauga Halton area or from primary care providers who practice in Mississauga, Oakville, Southwest Etobicoke, Milton or Georgetown * this includes referrals to both Ambulatory Outpatient Mental Health Services at Halton Healthcare and Trillium Health Partners

For all adult referrals 16 and over: for Mental Health and Addiction Supports provided through the Mississauga Halton use the standard referral form at www.one-link.ca

Halton referrals for Mental Health Services for Children and Youth: use the standard referral form at www.one-link.ca and your referral will be processed through one-Link to either the Child and Youth Outpatient Mental Health Service at Halton Healthcare or Reach Out Centre for Kids Access and System Navigation (ROCK ASN)

Mississauga and Peel referrals for WhereToStart.ca - Access to Mental Health Services for Children and Youth: use this referral form * these service providers can be reached at 905-451-4655 or faxed to 905-696-0352
