Service Description:
|
Transitional community based supports to individuals sixteen and older experiencing both mental health and substance use issues. The service offers quick access to community-based supports for those individuals who meet the criteria. Individuals can expect to receive assistance in navigating the mental health and addiction system, as well as the facilitation of appropriate referrals, transitional case management, and treatment planning.
