Service Description:
|
Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, registered nurses, registered practical nurses, pharmacists, nurse practitioners, dietitians, social workers, mental health counsellors, psychological associates, and administrative staff * services from the Family Health Team, including the weekend urgent care clinic are only available to patients who have a family physician at the Etobicoke Medical Centre * flu clinic is available for registered patients only * health education (virtual patient education classes and resources are provided via Zoom)
Urgent Care Clinic Hours: Sat 9am-3pm * Sun and Holidays 9am-12noon
Expanded services are available for patients who are enrolled with a Family Health Team family physician, including:
- injection clinic for B12 shots, allergy shots, flu shots, shingle vaccines, etc.
- registered dietitian services
- mental health and well-being services (416-621-2220 ext 253)
- clinical pharmacist services
- diabetes services
- anticoagulation service team (for patients taking warfarin)
- wellness for older adults

No restrictions but must be a patient of one of the physicians at The Etobicoke Medical Centre to receive expanded services from the Family Health Team
