Service Description:
|
In partnership with SHIP (Supportive Housing in Peel) to deliver housing support services to clients residing in SHIP units * SHIP provides affordable housing in a safe and supportive environment * program staff provides case management, service coordination, and person-centred support planning to SHIP residents
The overall goal of the program is to:- assist clients in achieving their individual support plans
- support clients and their families in their recovery journey
- advocate with landlords, hospitals, psychiatrists on client's behalf
- assist in developing the skills to improve quality of life, resolve day to day conflicts, manage setbacks and increase independence and housing stability

Adults 16 years or older with serious mental health issues * homeless or likely to become homeless * has residential, family or service links in Peel
