Service Description: Services include:

Primary care
  • services are provided by a team of family physicians, nurse practitioners, nurses and other medical professionals
  • services including care for children, prenatal care, minor procedures, gynecological care, adult health maintenance, cancer screening, and chronic disease management such as diabetes, high blood pressure
  • OHIP or IFHP (Interim Federal Health Program) is not required in order to access primary care services

Diabetes Education
  • program is designed to improve the quality of life of people aged 18 and older living with prediabetes, Type 2 diabetes, and those at risk of developing Type 2 diabetes
  • services are provided by a Registered Nurse and Registered Dietitian, through one-on-one counselling and group programs

Health Care Navigation and Support
  • focus on preventative care for all ages
  • proactive support to improve access to healthcare and wraparound social services
  • one-to-one and group programming are available
