The Find Your Family Doctor Team works directly with individuals and families in Halton to connect them to a primary care provider (family doctor or nurse practitioner) * health card or referral is not required- personalized support throughout the process
- help identifying available providers that match patient needs
- ongoing communication support
- interpretation services available
- wait times can vary depending on provider availability and client's specific needs
Clients can also register directly with Healthcare Connect by calling 811 or register online (to find a family doctor or nurse practitioner)
