Service Description: Provides personal support workers, rehabilitation services and nursing care for patients in homes, schools, retirement homes, long term care facilities, and workplaces

Services included:
  • Personal support: bathing, dressing, toileting, medication reminders, falls prevention, assistance with exercises and mobility, light housekeeping, other routines of daily activities * PSWs assist patients to get all the day-to day-things done that are essential for good health but that the patients may no longer be able to do safely for themselves * PSWs with additional training can perform skilled tasks such as administering insulin or using a glucometer
  • Nursing care: ostomy care, chemotherapy, chronic disease management, continence nursing, IV therapy, skilled shift nursing, pain management, enteral and parenteral nutrition, post surgical care, medication management, ventilator management, wound management, care of the fragile elderly, and care of medically fragile and technologically dependent children, health care teaching and home risk assessment
  • Palliative care: education and assistance with pain control * counselling regarding nutrition * help with symptoms such as nausea, shortness of breath or constipation; respite services * information and teaching on how to care for someone who is bedbound
  • Rehabilitation services: physiotherapy, occupational therapy, speech-language pathology, dietetics and nutrition, and social work
  • Caregiver support: safety supervision, social engagement, respite care, medication reminders, home management and home safety, overnight care

Fees: Service * fees are covered by OHIP if client is eligible for Ontario Health atHome services
