Service Description: A voluntary, time limited treatment program for emerging adults aged 16-24 who have prominent symptoms of mood, anxiety, psychosis or borderline personality disorder * focus is on providing stabilization and short-term treatment for emerging adults to ensure a successful transition from child and youth services to adult mental health services * team includes psychiatry, occupational therapy, social work, and focuses on providing stabilization and short-term treatment * services are provided to both patients and their families or caregivers * referral form can be found at one-Link

Services include:
  • comprehensive assessment and consultation
  • individualized treatment plan with a focus on stabilization, treatment and transition to community or adult mental health services
  • specialized individual and group treatment services offered by an inter-professional team
  • facilitation of referrals and community linkages

Fees: Medical services are covered by OHIP
Application: Medical referral is required * physicians can direct referrals to Centralized Intake at www.one-Link.ca
