Service Description:

Community-based hub designed to help older adults live well, maintain independence, stay connected, access resources, and remain safely in their homes and communities * services are delivered in partnership with local health and social service organizations, ensuring coordinated, person-centred care * 52-unit building for seniors that includes 22 supportive housing units on the second floor
Team works directly with individuals and caregivers to provide:- information and service navigation
- referrals to local supports
- advocacy
- on-site programs to promote overall health and well-being
Community Wellness Hubs run by Links2Care through the Connected Care Halton Ontario Health Team
