Service Description:
|
A charitable, not-for-profit, multi-service agency whose services help adults with a permanent disability and older adults live independently and safely in their homes * volunteer opportunities available
Services include:
Bathing assistance- provides one bath per week in the home at no cost to older adults or adults with disabilities to reduce the risk of a fall
- service is provided by personal support worker (PSW) in the client's home
Home help- staff provide housekeeping in the clients' home including vacuuming, cleaning kitchen and bathroom, laundry and grocery shopping
- companion/accompany to appointments via taxi/public transit (client covers cost)
- program fee is geared to income
Friendly Visiting and Calling- a volunteer visits isolated or lonely seniors and adults with disabilities in their home to provide companionship and conversation
- regular phone calls to older adults and people with disabilities who live alone, to check on their health and safety
Home Maintenance and Repair:
- fee for service program provides qualified workers to do jobs inside and outside of the home
- jobs may include installing grab bars, grass and yard maintenance, snow removal, painting, window washing or seasonal housekeeping, odd jobs, minor home repairs
Caregiver Recharge / Respite- relief for caregivers
- delivers flexible options to informal caregivers, providing worry-free in home relief with peace of mind that their loved one is being well cared for
- fee for service
- there are eligibility requirements to access this service that is provided at no cost
- respite program not available in Burlington
Community Navigation- support older adults or adults with disabilities who may be at risk due to a critical or impending change in their life situation such as an upcoming eviction, facing homelessness, loss of financial income, or experiencing difficulties within their home setting such as abuse and isolation
- Community Navigator will meet with people and/or caregivers to work to support achieving goals of financial stability, housing stability and linking to supports and services, completing required forms, etc.
- provides in-home assessments to identify needs, facilitate access to information and community resources and develop short and long term plans
- no cost for this service
Hot and frozen meals (available in Halton Hills only)- congregate dining * for older adults living in designated apartment buildings in Georgetown and Acton
- Wheels to Meals * held twice a month at a local restaurant, church or community hall providing an opportunity for seniors to get out and socialize with friends
- Meals on Ice * affordable frozen meals delivered by volunteers, or can be picked up at Links2Care offices
- Meals on Wheels * meals are delivered by volunteers at lunchtime Mon through Fri
