Service Description:

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness
- Family Medicine Teaching Unit (FMTU) is a joint partnership between Trillium Health Centre, the Summerville Family Health Team, and the University of Toronto (UoT)
- training for newly licensed doctors who are completing their post-graduate training in family medicine
- patients have access to nurses, nurse practitioners, social workers, a dietitian, a pharmacist and health promotion specialists
- currently accepting new patients * not accepting transfers of patients who already have a family physician
