Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, registered nurses, physician assistant, registered dietitian, certified diabetes educator, and other allied health professionals such as mental health counsellors, respiratory therapist, certified respiratory educator, chiropodist, physiotherapist, pharmacist, psychologist * focuses on chronic disease management, disease prevention and health promotion * all appointments with allied health professionals are booked through family physician's office
